Payments

For your convenience, FHR provides on-site insurance benefit verification.  With information from your insurance card and some personal data, our trained staff will obtain insurance benefits and notify you of your responsibilities and coverage prior to your appointment. All financial responsibilities can be discussed at this time.

We will submit all insurance claims for you as long as you provide all the required information. Required information includes:

Co-payments are expected at the time of your appointment. Cash, Check, Visa, Mastercard payments are accepted.

If you are experiencing financial difficulties, payment plans can be established.

Workmen’s Compensation/Auto Insurance
In order to obtain benefit verification and submit claims for work related injuries and auto injuries we need the date of the injury or accident, the place where the accident occurred and the claim number. In addition, we will need the name and number of the claims adjuster.

In Network Healthcare Programs
We participate with Medicare and Blue Cross and Blue Shield (all programs except Blue Choice). We will file and accept all insurance payments as payment in full after all co-pays, coinsurances and deductibles have been met.

Out of Network Healthcare Programs
For those who have insurances that we do not participate, you may choose to pay an out of network rate or choose to participate in a self-pay plan. Out of network benefits will be discussed with you prior to your appointment. Should you choose to pursue insurance coverage, you will be responsible for any co-pay, coinsurances and/or deductibles.

Self-Pay Patients
For self-pay patients, a discounted rate is available consistent with Medicare guidelines. Payment is expected at the time of your appointment.
